You have completed the Transferor section of this Transfer of Authorization. An email has been sent to the Transferee at the email address you provided. The Transferee will be invited to provide additional information that must be certified before this Transfer is approved by MDE.

You will also receive a copy of this email. Please forward it directly to the Transferee in the case that they do not receive it due to spam filters.

For individual permits, the transfer shall become effective upon issuance of a modified individual permit by the Department. For general permits, the transfer shall become effective after receipt of a completed Transfer of Authorization form, signed by both the transferor and transferee.
